Getting to Know My Child

Immunization Record or Signed Medical Exempt Form required prior to first day of care. Children are required to be fully immunized for our day care program unless it is for medical reasons (paperwork required by pediatrician).

If your child has a health care plan, please provide the plan to the day care.
